American Mary is a 2012 Canadian horror movie directed by the Soska Sisters also known as the Twisted Twins, Jen and Sylvia Soska. The film stars Katharine Isabelle, Antonio Cupo, and Tristan Risk. The movie follows the story of Mary Mason played by Katharine Isabelle, a medical student who is struggling to make ends meet. Mary is also a talented and skilled artist, but she cannot seem to find work that pays well enough. Desperate for money, she decides to take on an unusual job at a strip club, where she meets the club owner, Billy Barker played by Antonio Cupo.

Mary's life takes a twisted turn when she is solicited by a man named Beatress Johnson played by Tristan Risk to perform a medical procedure that will help her complete her transgender transformation. Encouraged by the amount of money she was offered, Mary agrees to perform the procedure. Shortly after, word of her skills as a surgeon spreads among the body modification community, and she starts to receive more and more requests.

As Mary becomes more involved in the body modification community she begins to lose herself, getting caught up in the underground world of extreme body modification. She meets some extremely eccentric individuals who seem to be addicted to the idea of changing their bodies in extreme ways. Mary is drawn to this world and is soon performing more surgeries on these individuals.

However, Mary's descent into this underground world has a dark side, and she starts using her surgical skills in a way that is not only unethical but also illegal. She uses her knowledge of the human body to grotesquely punish those who have wronged her, leading her down a dangerous and possibly deadly path.
