When the iconic British TV show "Doc Martin" premiered in 2004, it was an instant hit, and six seasons and several TV movies later, it is still beloved by fans around the world. The show follows the misadventures of Dr. Martin Ellingham, a successful London surgeon who moves to the fictional Cornish village of Portwenn to escape his high-pressure city life and pursue his dream of running a small-town medical practice. However, the irascible and socially awkward Martin quickly discovers that life in Portwenn is anything but quiet and idyllic, as he navigates a colorful cast of eccentric locals, medical emergencies, and personal conflicts.

In "Of All the Harbors in All the Towns," the fifth episode of season one, Martin (played by the talented Martin Clunes) finds himself facing a tricky medical case and a personal dilemma that threatens to upend his new life in Portwenn. The episode opens with Martin attending a meeting of the local council, where he is unceremoniously dismissed from his role as school doctor, leaving him angry and frustrated. However, his attention is soon diverted when he receives a call from his receptionist, Pauline, about a patient who has collapsed on a boat.

The patient in question is a fisherman named Jim Selkirk (played by actor Robert Blythe), who Martin quickly diagnoses with angina. As he treats Jim on the boat, Martin learns that the fisherman is struggling financially and is facing the possible loss of his boat, which is his livelihood. Martin offers to help Jim by writing a letter in support of his loan application and even offers to buy some of his fish, although his awkward manner and gruff demeanor make it clear that he is not exactly a natural at comforting people.

Meanwhile, Martin's personal life takes a complicated turn when he receives a visit from his ex-girlfriend, Louisa Glasson (played by Caroline Catz). Louisa, who is a teacher at the local school, wants Martin to talk to one of her students, a troubled boy named Peter Cronk, who has been acting out in class and claiming that he is sick. Martin reluctantly agrees, and after examining Peter, he discovers that the boy has a serious medical condition that requires surgery.

This revelation presents Martin with a dilemma - he wants to help Peter, but he is also concerned about the risks involved in the surgery and the potential for a lawsuit if something goes wrong. Martin also has to deal with his own awkwardness around children, which is not helped by the fact that Peter's mother is a difficult and overprotective woman who does not trust Martin's judgment.

As Martin tries to navigate these challenges, he also has to deal with the messy aftermath of his brief fling with a local pharmacist named Elaine (played by Lucy Punch), who is determined to pursue a relationship with him despite his lack of interest. Martin's attempts to distance himself from Elaine lead to some uncomfortable confrontations and a classic scene where he flees from her advances by hiding in his own office.

Despite these personal and professional obstacles, Martin manages to save Jim's boat, help Peter get the medical care he needs, and even make some progress in his own emotional journey, thanks to some blunt advice from his Aunt Joan (played by veteran actress Stephanie Cole). Through it all, "Of All the Harbors in All the Towns" showcases the mix of dry humor, medical drama, and quirky characters that have made "Doc Martin" a beloved fixture of British TV and a cult favorite around the world.
