Sam and Dean investigate a string of murders in a small town. As they investigate, they discover that the victims all died from heart attacks even though they were otherwise healthy. Sam and Dean also find out that all of the victims were involved in some sort of secret affair. As they continue to investigate, Sam and Dean begin to suspect that a siren is the culprit behind the murders.
The brothers enlist the help of a local deputy named Jenna, who has a history with Sam. Jenna initially refuses to help the brothers, blaming them for the death of her father. Nevertheless, Sam and Dean manage to convince Jenna to aid them in their investigation.
Meanwhile, Castiel is struggling with his own personal problems. He discovers that he has been infected with a spell that compels him to kill any angel who comes into contact with him. Desperate for a cure, Castiel turns to Crowley, the King of Hell, for help. Crowley, however, is hesitant to get involved, as he and Castiel have an uneasy history.
As Sam, Dean, and Jenna search for the siren, they discover that it has taken up residence in a nearby hotel. They also discover that the siren has been manipulating its victims by making them fall in love with it. The siren then feeds on the victims' love, ultimately leading to their death from a heart attack.
Sam and Dean confront the siren and attempt to kill it, but to no avail. Meanwhile, Castiel tracks down a potential cure for his spell in the form of another angel. However, in order to get the cure, Castiel must fight the angel in a brutal battle to the death.
As the battle rages on, Sam and Dean continue to try and kill the siren. In a dramatic climax, they manage to subdue the creature and trap it in a box. The brothers are then faced with a difficult decision: whether or not to kill the siren, knowing that there may be unintended consequences.
The episode ends with Castiel managing to cure himself of the spell and Sam and Dean taking the trapped siren with them on the road, unsure of what they will do with it next.
