A brilliant young doctor named Grace Devlin (played by Jordana Spiro) finds herself caught between two worlds in the pilot episode of The Mob Doctor. On one hand, she is a talented surgeon working within the strict confines of a major Chicago hospital. On the other hand, she is also the daughter of a notorious local crime boss, Constantine Alexander (played by William Forsythe). When her brother Nate (played by Jesse Lee Soffer) incurs a dangerous debt to the mob, Grace is forced to make a fateful decision.

The pilot opens with a tense operating room scene, as Grace frantically works to save the life of a gunshot victim. The plan involves performing a secret surgery on a mob boss named Paul Moretti (played by Michael Rappaport), who is a patient at Grace's hospital. Moretti is suffering from a rare form of cancer, and requires treatment that only Grace can provide. But in exchange for her help, Moretti demands that Grace become his personal physician, and attend to his medical needs -- and those of his criminal empire -- whenever called upon.

Grace reluctantly agrees to this deal, but quickly realizes the enormity of what she has gotten herself into. She must now juggle her demanding job at the hospital, her obligations to her family, and her dangerous new role as the mob's personal doctor.

Meanwhile, Grace's medical colleagues begin to notice that something is off about her behavior. Her best friend, nurse Rosa (played by Floriana Lima), senses that Grace is hiding something, and tries to get to the bottom of it. Another colleague, Dr. Stafford White (played by Zach Gilford), becomes suspicious of Grace's loyalty to the hospital, and starts to investigate her past.

As the pilot episode comes to a close, Grace finds herself in a precarious position, with enemies closing in on all sides. She must use her wits and her medical skills to stay one step ahead of her rivals, all while keeping her family and her colleagues in the dark about her dangerous double life.
