Four years ago Officer Frank Williams (Steven Vidler) and his partner are called to an abandoned house where they find a woman who has been the victim of a brutal attack, and her eyes removed. Upon discovering her, Frank’s partner is attacked with an axe and murdered, and while Frank survived the assault he is left with a severed arm. Later detectives find seven murder victims in the house, all with their eyes removed. In present day, Frank is working as a guard in the county’s detention center, and he is given an assignment to take several convicts to an abandoned hotel for cleanup work after a fire. The hotel’s owner, Margaret (Cecily Polson), informs the group that the hotel is going to be converted into a homeless shelter. In exchange, under Frank’s supervision, the convicts will receive reduced sentences. Little does the group realize, but Jacob Goodnight (WWE superstar Kane) has been hiding out in the old hotel. The group divides up, each on their own missions. Tyson (Michael J. Pagan) and Richie (Craig Horner) set out to find the owner’s safe, which was never found after the fire, but instead they discover a dead body. One by one, the group encounters the psychopath and all hell breaks loose and no one is safe. Frank realizes that the killer is the same one he encountered four years ago, and it is no coincidence that he ended up at the hotel. Flashbacks to Goodnight’s childhood filled with violent religious rituals bring some clarity as to why he is a vicious killer. The group soon realizes that someone in their midst has arranged the bloodbath, is helping Goodnight with his sacrifices and has no intention of allowing them to leave. In the end, the remaining survivors realize that there is only way out and they must face him head on with eyes wide open.
