After binge-eating and binge-drinking for days, Sweet Dee's (Kaitlin Olson) unhealthy lifestyle finally catches up with her, and she has a heart attack. The gang, including her twin brother Dennis (Glenn Howerton), her friend and business partner Charlie (Charlie Day), her ex-husband Mac (Rob McElhenney), and their friend Frank (Danny DeVito), rushes to the hospital to visit her.
At first, they seem genuinely concerned about their friend's health, but as soon as they realize that Dee's hospital bed is more comfortable than their own couches, they begin to take advantage of her situation. They come up with all sorts of schemes to make themselves comfortable in her hospital room, from using her side of the room as a storage space for their belongings to eating her hospital food.
Meanwhile, Dee is struggling to make sense of what happened. She tries to blame her heart attack on external factors, like bad genes or her stressful job, but her doctor (Mary Elizabeth Ellis) tells her the truth: her poor lifestyle choices are what put her in this situation. Dee is forced to reevaluate her life choices and come to terms with her own mortality.
Throughout the episode, we see flashbacks of Dee's unhealthy behavior. She eats nothing but junk food, drinks heavily, and smokes cigarettes. She neglects her health and her job, and pushes away those who care about her. Her heart attack is a wake-up call, and she finally begins to take responsibility for her own well-being.
As the gang continues to lounge in her hospital room, Dee's doctor becomes increasingly frustrated with their behavior. She tries to kick them out, but they won't leave. Things come to a head when Dee's ex-boyfriend (Jason Sudeikis) comes to visit her, and the gang mistakes him for a doctor. They reveal all sorts of embarrassing secrets about Dee, including the fact that she once had sex with a dog, and hilarity ensues.
Ultimately, Dee's heart attack leads to some much-needed character development for her and the rest of the gang. They learn to be more responsible and caring, and Dee realizes that she needs to make some serious changes in her life. But even in the face of adversity, the gang can't resist their own selfish impulses, and they continue to make the most of their time in the hospital room.
