Detectives Andy Sipowicz and John Clark Jr. investigate a robbery and shooting at a convenience store, where the store owner, a single mother, was killed. They soon discover that the victim had undergone a mastectomy and was still in the process of reconstructive surgery, causing them to speculate that the robbers mistook the woman for a transgender person. The case leads them to a transgender support group where they interview several members, including a woman who discovers she has cancer and may need to undergo a mastectomy.
Meanwhile, Detective Medavoy's ex-wife pays him a visit, informing him that his daughter has left college and is now living with her boyfriend. Medavoy is furious and visits his daughter to convince her to return to school and avoid making the same mistakes he did in his youth.
Elsewhere in the precinct, Detective Diane Russell struggles with a frustrating case involving a man who deliberately caused a car accident to collect insurance money. Her investigation leads her to the man's wife, who is hiding a dark secret and may have been an accomplice to the crime.
As the detectives work their cases, they face personal challenges and struggles, making for a thought-provoking and emotional episode of NYPD Blue.
