The Case Study of Vanitas takes place in a world where humans and vampires coexist, but the latter are forced to wear special collars that limit their powers. The show follows Noé, a young human who once met a man named Vanitas and became fascinated with his book, The Book of Vanitas.
In the first episode, Noé is on a mission to find the Book of Vanitas again. He visits a mansion in Paris where he believes the book might be located. However, he gets attacked by vampires who are trying to protect the mansion, causing a ruckus in the streets.
Meanwhile, a young vampire named Jean heads to the same mansion with his master, Dominique, to attend a ball. Jean is curious about the mysterious Vanitas and his book, but Dominique warns him to stay away from him and his work. However, during the ball, Jean feels unwell and his collar starts to malfunction, causing him to reveal his fangs. The other vampires in attendance start to panic and turn on him, but he is saved by none other than Noé.
Noé and Jean start to bond over their interests in Vanitas and his book. Jean reveals that his grandfather, who was a scientist, had worked with Vanitas and even created his book. However, he was also branded a heretic and his work was discredited. Jean is hesitant to believe Vanitas's claims that the book can cure all vampires of their curses, but Noé is more optimistic.
The episode culminates in a confrontation between Noé, Jean, and the vampires who are protecting the mansion. Just when the situation seems dire, a man in a top hat and scarf appears and introduces himself as Vanitas.
