Meanwhile, Heo Im's past catches up with him when he is visited by a member of the royal family. The family member tells Heo Im that his past actions could have consequences for him in the future. Heo Im is left worried about his future, but he does not let it distract him from his work at the hospital.

Later in the episode, Heo Im is faced with another difficult case. A young boy is admitted with a severe head injury after being hit by a car. Yeon-kyung tells the boy's parents that there is little hope for their son's recovery, but Heo Im refuses to give up.

Heo Im spends hours performing a risky surgery on the boy, and his determination pays off when the boy's condition stabilizes. Yeon-kyung is impressed with Heo Im's dedication, but she wonders if he is putting himself and his patients in danger by using outdated techniques.

The episode ends with Heo Im reflecting on his experiences in modern-day Korea. He is grateful for the opportunity to learn from Yeon-kyung and her colleagues, but he realizes that he still has much to learn.
