Despite being warned not to go to the orphanage as he may put his life in danger, Tenma decides to visit the place in order to confront Johan.

Johan then explains his connection to Kinderheim 511, describing his experiences at the orphanage and the horrific experiments that were performed on the children. The episode follows the story of Johan and how he became involved with the Red Rose Mansion, as well as how he met his sister Anna whilst living at Kinderheim 511.
Throughout the episode, Johan's intentions become harder to gauge, as his words seem to contradict his actions and previous behavior. The viewer is left unsure whether to trust Johan or Dr Tenma, as their motivations for their actions are not made entirely clear.
