Bohannon is still on the hunt for the men who killed his wife and son, and he's getting closer. With the help of Elam, he tracks the group to a nearby Indian village, where they plan to sell the stolen horses. Bohannon and Elam confront the men and a brutal fight ensues, leaving several dead on both sides.
Meanwhile, Durant is facing a crisis of his own. The railroad is behind schedule and over budget, and he's under pressure to get it back on track. To make matters worse, a group of Sioux Indians have been attacking the work crews, slowing down progress even more. Durant makes a desperate deal with one of the Indian leaders, but the truce is fragile.
Back in town, Doc is running the hospital and dealing with a horrific outbreak of smallpox. He's short on supplies and staff, and the disease is spreading quickly. To make matters worse, one of the workers on the railroad is infected and has to be quarantined. Eva tries to help out as best she can, but tensions are running high and people are scared.
The episode also features a subplot involving Lily and Joseph, two former slaves who are now working as prostitutes in the town. Joseph becomes increasingly violent and possessive, leading Lily to seek help from a sympathetic Madame.
As the various storylines converge, tensions come to a boiling point. Bohannon and Elam return to town with their prisoners, but Durant is furious that they've been distracted from the railroad. The smallpox outbreak continues to spread, and there are signs that the truce with the Sioux may not hold. And as Lily and Joseph's situation reaches a breaking point, someone may end up paying the ultimate price.
