Allison Dubois, a wife, mother and aspiring lawyer has an extraordinary ability to communicate with the dead. In 'Being Mrs. O'Leary's Cow', the thirteenth episode of the first season of the series, Allison is presented with a complicated case that hits home. The episode opens with Allison dreaming of a field filled with cattle. Among the cows, she sees a pale and sickly cow who is crying out for help. She sees a vision of the cow's owner, Mrs. O'Leary, and subsequently awakens with a sense of dread. Allison wakes up to her husband Joe in bed, asking how her night was, all while she is trying to shake off her vision.

At work, Allison's assistant Lynn arrives with a new case file. Allison is not entirely pleased with the new file and cautions her assistant to make sure that the team examines all of the material before agreeing to help.

Allison and her husband Joe Dubois invite a new couple, Bob and Sara, to come to dinner to meet with Bob's bosses and his wife. The work arrangement is headed by a woman named Mrs. O'Leary, who just happens to be the owner of the same cow in Allison's vision. Bob is under immense pressure to sell his product to the company, but when the food is served, he collapses, and the Dubois family is called in to help once more.

Things become more complicated when Allison realizes that Bob's falling ill was not an accident, but indeed an intentional act. Allison struggles to get the bottom of the case, so she seeks more insights from her friends, D.A. Devalos and his associate, Lee. Allison also seeks help from her dreams with her dreams becoming more and more an integral part of the case.

As the episode progresses, Allison has a new dream, and she sees a man burning a barn down. Her dreams are indicating to her that the arsonist is powerful and unfortunately has not yet been caught, but who could it be? Could it be someone involved in the O'Leary family? As she continues her investigation, she hears a phone call with the arsonist speaking of the fires planned, and she becomes even more determined to catch this arsonist.

The episodes take a wild turn when Mrs. O'Leary's daughter's life is put at risk when the arsonist strikes again. Allison's husband, Joe, acts as the lead investigator in the case, and they realize that the arsonist is one that they all know. It isn't until Joe uses his investigative skills that they finally catch the arsonist before anyone else is harmed. Mrs. O'Leary is relieved that her daughter is safe and is free to carry on with her life.

The episode concludes with Allison savoring a hard-won victory with her family after the days of investigation. They share a moment of triumph with each other and enjoy each other's company. These moments are rare for Allison, who is always juggling with work and family life, but in this victory, she feels like her world is complete.

The show Medium, in Being Mrs. O'Leary's Cow, once again highlights Allison's psychic abilities, cementing its popularity as a series that tells the story of a woman dealing with life and family while grappling with extraordinary abilities. The episode keeps the audience on their toes with its twists and turns, with the themes of the episode revolving around family, loyalty, and trust.
