Jack's Back is a mysterious crime movie, and the directorial debut of Rowdy Herrington who also wrote the film. In the film, a serial killer is loose in Los Angeles. Unfortunately, homicide police detectives believe the killer is a Jack the Ripper copycat killer. In fact, the new serial killer is targeting prostitutes and using the same method to kill as the original Jack the Ripper. In addition to the similarities, it is also the 100th anniversary of the Jack the Ripper slayings, and cops believe this is what activated the murder spree.

With very little to go on, the police detectives begin to suspect that the serial killer is either a medical student or a doctor. When a young, naive surgeon John (James Spader) becomes a prime suspect in the murders, he is quickly eliminated. Unfortunately, John is only taken off the suspect list because he too became a murder victim.

Immediately after John's murder, John's twin brother, Rick (James Spader), steps forward with information for the police detectives. Rick states that his brother was killed simply because he was in the wrong place at the wrong time. When police ask him how he knows this, Rick claims he had "visions" of his brother's murder.

Instead of taking Rick's information seriously, the cops immediately suspect Rick in his brother's murder, and the murder of the prostitutes as well. After learning he is a suspect in the copycat murders and his identical twin brother's murder, Rick sets out to clear his name, and catch the real killer for the police.

However, catching the killer may not be an easy task for Rick, especially if the police arrest him and book him with the heinous murders before he can finish his independent investigation.
