Johan Liebert, a former patient of Dr. Tenma, returns to his childhood home in Heidelberg, Germany where he reunites with his twin sister, Anna. As they catch up on lost time, Johan reveals his true identity and the atrocities he has committed since leaving the hospital. Meanwhile, Inspector Lunge is in pursuit of Johan, following a lead to Heidelberg. As he investigates, he discovers the true extent of Johan's handiwork and the danger he poses to society. Dr. Tenma also arrives in Heidelberg, determined to put an end to Johan's reign of terror once and for all.
