The Village Idiot (J.D) is summoned to help Princess Elliott when she cannot figure out what is wrong with her handmaiden, with the help of the Giant (Janitor) and a little magic from Turla (Turk and Carla) and last but not least the Knight in shining armor of this fairytale (Dr. Cox) they may be able to defeat the Dark Lord (Dr. Kelso) and save the handmaiden.

Elliott and J.D gets in touch with an old patient of theirs to offer him some compensation as the medication they recommended had some bad side effects. Carla speaks to Turk about getting pregnant again while Dr. Cox likes his new title, but does he want all the responsibility and problems that comes with it?

When Dr. Cox is visited by one of his regular patients something strange happens, he can't find out what is wrong with him, so J.D and Turk immediately try and find out what is wrong with Dr. Cox's patient in order to once be better then him. Elliot and Carla sets out on a mission to get the board to let Dr. Kelso stay on as Chief Of Medicine while Dr. Kelso himself thinks about his time at Sacred Heart.

J.D. and Turk are both facing the same dilemma, appearing masculine. J.D. wants to set a manly example for his son, while Turk undergoes a surgical procedure that causes him to feel insecure. Meanwhile, a disagreement with Kelso has the Janitor creating a hospital newspaper.

Sacred Heart hospital is signed up for RateYourDoctor.org. a website where patients are able to grade their doctors, by Dr. Kelso. The use of the website does not have the effect Dr. Kelso was aiming for, now the doctors are just aiming to be number one. Elliot is having a dilemma when it is revealed to her that one of her terminal patients intentionally overdosed on medicine. Meanwhile the Janitor has a new girlfriend, and Carla tries to figure out what she sees in him.

J.D. decides to grow up in order to better care for his son, but when he stops engaging in Turk's antics, Turk brings it upon himself to bring back J.D.'s immaturity. Meanwhile, Eliot decides to throw Kelso a birthday party, but when his age is revealed, big changes are made at Sacred Heart.

Carla worries that she's losing her Latin identity and fears that her daughter may never learn about her heritage. J.D. lands himself in hot water when the staff realizes that he uses nicknames because he doesn't know anyone's real name. Upon hearing this, the Janitor proposes a challenge -- J.D. must identify everyone by name in the hospital, and if he can't he has to be a janitor for a day.

When the Janitor watches "An Inconvenient Truth," he decides to become an Enviornmental Officer (with peculiar motivational methods) at Sacred Heart. J.D. must deal with an inconvenient truth of his own when Dan returns to town and tells him he has to grow up.

Kim goes into labor and both she and J.D. begin to worry about their future together as well as being parents. Dr. Cox refuses to give his daughter a shot, thinking she'll always associate her father with pain; Turk and Carla try to complete a video game so it doesn't distract them from being good parents; and Dr. Kelso helps his son recover from a broken heart.

After ending up in a compromisng position, J.D. rethinks his committment to Kim and Elliot contemplates her pending marriage to Keith. Carla agrees to let Turk choose one candy bar to help control is sweet tooth. Meanwhile, Dr. Cox has to solve a patient's mysterious ailment and the Janitor shows off his Lady.

The christening of baby JD, Dr Cox an Jordan's daughter, is all that JD can think about until they replace him as godfather. Elliot has doubts about her marriage with Keith, who is being lectured by the brain trust on how to be a good husband. JD still has doubts about his relationship with Kim and he and Elliot end up in the on call room all alone.

JD struggles to decide whether he will forgive Kim and stay with her or not and Carla struggles to prepare a party for Elliot, who proves to be an enormous control freak.

As Eliot begins to plan her wedding with Keith, J.D. continues to question his true feelings for her. In an attempt to ease his best friend's mind Turk takes him to a surgeon's convention in Phoenix, but the plan backfires when J.D. runs into his ex-girlfriend Kim and comes face to face with the unexpected. Meanwhile, Kelso and his cronies party it up at the convention while unknown to him, the Janitor takes on the role of chief of medicine back at Sacred Heart.

When Eliot's engagement ring doesn't fit, Keith agrees to a "redo" in which the particular Doctor plans her own engagement. Carla, Jordan and Eliot's sorority sister Melody gather to help plan the "surprise." Meanwhile, the men of Sacred Heart are suffering from too much talk and not enough action as J.D. continually gets shot down by Melody, Turk and Carla have not been intimate since their baby was born and Dr. Kelso's cold showers. In the end J.D. realizes that he might still have feelings for Eliot.

J.D. feels a little left out when Eliot's wild sorority sister, Melody comes to town. Not able to deal with being the third wheel he turns the girls against each other in the hopes that he will once again be Eliot's priority friend and, if he is lucky, that he might get to "mash" it with Melody. Meanwhile, Turk and Dr. Cox go head to head when they disagree over whether or not a patient requested surgery is necessary. Elsewhere, Carla discovers an unlikely ghost haunting pediatrics in an attempt to keep it clean. In the end one of Sacred Hearts own will be considering a marriage proposal.

In a unique twist, J.D. turns his narrating duties over to Ted, Todd and Jordan -- as this episode focuses on the lives of the supporting players at Sacred Heart Hospital. Ted juggles fantasies in which he has a full head of hair and a reality where he is helping the nursing staff fight for a raise; Todd meets his future son and helps Turk preserve his reputation and Jordan puts aside her knack for causing misery to help Elliot and Keith's relationship.

After saying goodbye to Nurse Roberts, the Sacred Heart staff institutes some of her lessons into their lives. Though Carla thinks they've all forgotten, Dr. Cox takes time to reflect on his purpose; Elliot opens her heart to Keith; and J.D. and Turk learn compassion.

Nurse Roberts' condition begins to worsen and each of the staff members deal with the possibility that she may not make it in their own, individual way. Elliot and Keith cope by staying busy together, J.D. and Turk deal through childish behavior, and Carla finds comfort in an unlikely source. But as one life may end, Dr. Cox and Jordan welcome a new life and J.D. tries to sell himself as the best choice for the baby's godfather.

Turk hires a hot nanny and though both Turk and J.D. are pleased with her, Carla is not. Elliot and the Janitor due their best to cheer up a dying patient, but Dr. Kelso isn't thrilled with their enthusiasm. Meanwhile, Nurse Roberts and Dr. Cox debate whether or not "everything happens for a reason".

Unbeknown to Dr. Kelso, J.D. and Turk try to beat the system to treat the Chief's uninsured friend. Elsewhere, the Janitor blackmails Carla with a case of mistaken dog identity. Meanwhile, Dr. Cox tries to convince Eliot that a reformed drug addict isn't quite clean. All the while the entire staff is condemned to wearing dirt brown scrubs when Kelso suspects a clothing thief in their midst.

The day has come for Private Brian Dancer to be released from Sacred Heart, but when the military informs him that his injuries are too severe to return to service he spirals into a state of depression. Eliot, J.D. and Turk rally around him to boost his spirit. Meanwhile, Dr. Cox informs Carla that her jokes are just not funny.

While helping a patient regain his memory, the staff at Sacred Heart stir up some memories of their own. J.D, Elliot, Turk, Carla, Dr. Cox, Dr. Kelso and Janitor take a trip down memory lane in this unique look back on six years of 'Scrubs.'

J.D. falls for a sexy new physical therapist but because of his unfortunate circumstances, he finds it difficult to ask her out. Meanwhile, Turk breaks his arm and Dr. Kelso sends him back to residency where, with the help of Dr. Cox, he realizes that he knows less about actual medicine than he thought he did. Elsewhere, Eliot invites Keith to move in with her, but isn't quite ready to compromise her house rules. All the while Carla provides each of the doctors with guidance while they get over their fear, ignorance and stubbornness.

When J.D. develops vasovagal syncope, a condition which results in fainting or blackouts, he finds himself not only having to deal with losing his girlfriend and his apartment, but also his consciousness. Eliot and Turk debate over whose turn it is to take care of the always down on his luck resident, while Carla tries out her new excuse "I have a baby." Elsewhere, the Janitor embarks on a cruise vacation...or does he?

Turk convinces J.D. that he needs to go to Tacoma for Kim's first ultra sound --which results in an impromtu road trip for the Sacred Heart crew. After commandeering Dr. Kelso's new RV Eliot, Carla,Keith, Ted, and the Janitor and the father to be on his journey. Meanwhile Dr. Cox and Jordan face some concern when her ultrasound reveals that her unborn child will need to have surgery.

While the staff questions Dr. Kelso's commitment to the hospital we begin to see the world from his point of view. But when he befriends patient, Private Brian Dancer, the war stories he begins back from Iraq reminds Kelso what it means to be a good leader. Meanwhile Dancer's presence in the hospital causes a heated political debate amongst the staffers dividing them down party lines, while everyone is fighting the patient's well -being is being neglected.
Elsewhere, J.D. is in search of a new apartment and feels out of the loop when politics becomes the topic of the day Sacred Heart.

Sacred Heart is turned into a full scale Broadway stage when Patti checks in complaining of hearing incessant music. While trying to determine the cause of this strange complaint the residents of Sacred Heart star in their own musical. Carla and Turk tango over her decision to be a stay at home mom or to return to work, while Elliot struggles with telling J.D. that she doesn't want to be roommates with him anymore.

Elliot enjoys pimping out the apartment with the extra money from her new private practice gig and J.D. is enjoying the spoils, but when his jealousy surfaces he loses his new 60-inch plasma TV privileges. Elsewhere, Dr. Cox, in an attempt to hide from Jordan, bunks with the Janitor in one of the hospital's luxury suits - but the happy roommates soon turn on one another and their rivalry escalates until only Dr. Kelso (Ken Jenkins) can save them. Meanwhile, Carla is still struggling to admit that she may have postpartum depression and Turk encourages her to seek help.

Dr.Cox channels TV doctor 'House' to solve three mysteries including; why a man's skin is inexplicably orange, why a young woman, with no coronary issues has a failing heart and why Elliot is taking her anger towards Dr. Kelso out on Turk. But the doctor may be missing the biggest clue of all when Carla begins to show signs of postpartum depression. Meanwhile, J.D. tries to be the 'best boyfriend ever' so that Kim won't take the job in St. Louis

The expirienced surgeon Dr. Turner is going to lay down on the operation table. Elliot tries to convince him that medicine is a better option since surgery could kill him.

Carla goes into labor and an excited Turk strikes up the band -- literally -- while escorting her to the hospital, but he still feels upstaged by Elliot's attempts to make Carla more comfortable. A scheme to make himself look like the hero backfires and Turk is in jeopardy of missing the birth of his child. Lucky for him the whole thing is mistakenly being broadcast over the hospital's closed circuit TV. Meanwhile, J.D. and Kim contemplate their future and the future of their child.

JD is still reeling from the news that he is going to be a father. Meanwhile, Dr. Cox -- while readying himself for baby number two -- realizes that his constant anger may have future repercussions. Elsewhere, Turk and Carla prepare for the arrival of their little one, while Elliot laments that she is the only one in the hospital not having a baby! They all begin to see a little of themselves in their patients ailments, but will they find the cures?

J.D. prepares for a perfect date with his new flame, Kim. The hospital staff discusses about a name for Turk and Carla's baby and is surprised that also somebody else is expecting a baby.

J.D. is consulting an urologist about a possibly upcoming surgical procedure on one of his patients. Elliot considers a breakup with Keith, because have any grit, especially when Dr. Cox is takes up on him.

For J.D. things seem to be on repeat after the five yers in the hospital. After his alcohol excess Dr. Cox seem to be unsure about ordering treatments for his patients. Carla gets annoyed by Turk telling her stuff she can't do because of her pregnancy.

Dr. Cox turns to drinking and doesn't show up in the hospital after he made a decision that results in the death of three patients.

While working to stabilize three patients awaiting organ transplants, J.D. resolves to ask Dr. Cox to lunch until he agrees. When he finally chases Dr. Cox to lunch, J.D. runs into annoying former patient Jill Tracy, who unexpectedly teaches him something about responsibility. Later, Elliot and Carla discover that Todd's been lying about all his hospital "conquests" and suspect that he's been less than truthful about other issues, but Turk warns them not to meddle.

After J.D. helps Turk and Carla turn his old bedroom into a nursery for the baby, he gets the idea to strap a digital camera to his head so his mom can see life through his eyes. But the project is cut short when the Janitor seizes an opportunity to shove J.D. out of the picture for a day. Meanwhile, when mistakes by the interns begin to reflect badly on her, Elliott cracks the whip on her charges and even resorts to using her mother Lilly to lean on Keith for information about his peers. Later, Turk accuses Dr. Cox of not being hip.

When J.D.'s underachieving brother Dan returns for a surprise visit, Elliott learns that J.D. has been telling Dan that she's still into him, but when the truth comes out, Dan turns to the Janitor to get back at J.D. As everyone reacts to the pregnancy in different ways, Carla and Turk begin to discuss baby names, and despite his promise to Carla, Turk is unable to keep quiet about their initial choices. After assigning someone else to assist "sweaty Teddy" with incoming malpractice claim paperwork, Dr. Cox is caught off guard when Dr. Kelso steps in to stick him with the job. Meanwhile, a patient with weight-related problems needs an MRI, but the staff has to come up with a solution when they find he's too big for the machine.

J.D. changes his night off so he can hang out more with Turk, but Turk is sad to lose his own personal "Turk Night". Meanwhile, Jordan forces Dr. Cox go on a double date with Elliott and Keith. Dr. Kelso asks Carla to cover for him and turn down staff requests while he deals with the death of his dog. Later, when a patient almost dies after giving his brother part of his liver, the gang learns that sometimes they have to do things they might not like for the people they care about.

Turk accidentally learns that Carla is pregnant before she does, so J.D. convinces Turk to tell the entire hospital staff so they can all surprise Carla with the joyous news. But when Carla confides to Turk that she can't wait for the day when she can personally share her baby news with the world, Turk and J.D. have to quickly figure out how to contain hospital gossip until Carla can make the discovery on her own. Meanwhile, J.D. tricks the Janitor into swallowing a tiny GPS tacking device. Later, when Elliott puts Dr. Cox into an embarrassing situation, he decides to get even with Elliott through Keith and Carla.

When Dr. Cox scoffs at reading lines with a patient preparing for a play, J.D. urges Cox and his fellow doctors to do special, unconventional things for their patients, but when J.D. scores a date with a hot babe who loves his hair, he's not sure he can shave his head for a woman undergoing chemo. Turk and Carla attempt to eliminate all the stress in their lives to better their chances of conceiving, but Turk has to compete for a surgical resident spot and Carla loses Dr. Kelso's VIP patient. Meanwhile, Elliot showers new boyfriend Keith with stupid gifts.

J.D. is sick of how perfect intern Keith is and he's not the only one. As hospital gossip spreads and Elliot's relationship with Keith is revealed, the other interns think he is getting special attention. Meanwhile, seeking some special attention of his own, Dr. Cox boasts about being named "The Best Doctor in the City" by a local magazine, but Dr. Kelso deflates his ego and assigns him to the most difficult patient in the hospital. Elsewhere, Turk and Carla continue to struggle in their quest for conception.

J.D. and Dr. Cox grow close as they go through the five stages of grief with their favorite patient, who is declining in health. Meanwhile, Carla forces Turk to go on a string of double dates, one of which includes Elliot and her booty call Keith. And when Dr Kelso runs Ted down with his car, Ted finally gains power over Kelso with a potential lawsuit.

Dr. Kelso is outraged when he discovers that a bird has been living in Sacred Heart. Citing the spread of infection to be the number one cause of deaths in hospitals, Kelso, orders the Janitor to dispose of it immediately. However, the Janitor likes the bird so much he secretly keeps it as a pet. J.D., annoyed that Elliot is hooking up with Keith, tries to get his intern fired.

J.D. is still dealing with his recent break-up and trying not to kill intern Keith, who has become annoyingly perfect. To make matters worse a favorite patient is in a serious coma and each of the staff finds their own way to deal with it. Dr. Cox and the Janitor bond over drinks at the bar and Carla makes it her mission to make the hospital's gym female friendly. Meanwhile Turk suggests that J.D. and Elliot find themselves 'booty calls.'

J.D. becomes annoyed that new girlfriend, Julie, doesn't laugh, but rather tells him when things are funny. Afraid that he is going to screw up yet another relationship, Elliot encourages J.D. to start saying his feelings out loud and Turk tries to use his psychology background to fix the issue. Meanwhile, Carla is upset when the Janitor refers to the new nurse as a "hot young Carla", increasing her growing concern that she is too old to conceive.

J.D. is introduced to quirky Julie Quinn and love is in the air. Worried that J.D. will once again screw up the budding relationship Elliot doles out effective romance advice. Meanwhile, Carla questions Dr.Cox's tough exterior and is convinced that the sarcastic doctor is really a softy. Elsewhere, the Janitor puts together an air band to win a contest.

J.D., Turk, Elliot and Carla are asked to recount the past weeks events during and interrogation surrounding the mysterious death of a patient.

But Dr. Kelso will find it hard to pin point neglect when he learns that J.D. and Turk were away tracking down a patient to illicit a thank you, and Elliot was traumatized over finding out that her make-out buddy was married and Carla was busy recruiting hospital personal for a group lottery purchase.

J.D. is called in to work on his day off and every time he tries to go home, he is pulled into another drama. Turk searches for a heart donor to save a dying patient, and Carla is worried that she doesn't have the courage to become a parent.

Meanwhile, Elliot doesn't think she has the brains to lead a question and answer session on a subject she knows little about. All the while the Janitor is trying to make the hospital easier to navigate by painting color-coded lines, including a yellow line, which leads to the exit.

J.D. mistakes an elderly woman's satisfaction of leading a good life as her wish to stop treatment for a fatal illness and the patient in turn chooses to work with Dr. Cox.

Meanwhile, Turk and Elliot struggle to find the cause of a patients unexplained yet excruciating body pain and Carla struggles with the uncooperative Sacred Heart staff when she tries to organize the annual group photo.

Dr. Cox's religious sister Paige (guest star Cheryl Hines) comes to town for her nephew's baptism and challenges her brother's faith.

Meanwhile, Turk is finding out just how unsexy baby making can be. It takes a heart to heart with Elliot for him to realize that Carla finds the whole process to be romantic. Elsewhere, J.D. and the Janitor seem to have a budding friendship.

When J.D. is asked to introduce Dr. Kelso at an awards dinner, he sets out to discover something good to say about the grouchy doctor.

Meanwhile, Dr. Kelso and Dr. Cox each have a patient whom they feel deserves the benefit of a new experimental treatment, but a moral standoff ensues when there is only one open slot in the program.

Elsewhere, Carla and Turk try to convince Elliot to let them help her get her old job at Sacred Heart back after they discover that she's been working at a free clinic. All the while, the Janitor taunts J.D. with the made-up game of Jiggly Ball.

As J.D.'s 30th birthday approaches, he sets out to accomplish one item on his "Things to Do By Thirty" list: complete a triathlon. Meanwhile, Elliot urges Jake to open up about his wants and needs, but immediately regrets it when he does. Turk lets Carla believe she's responsible for his decision to perform hypnosis surgery, but he's really doing it to impress Dr. Kelso.

J.D. questions the bond he has with his interns when Dr. Kelso tells him that inevitably he will begin treating them badly, as it has been throughout history at the hospital. The Janitor further complicates matters by telling J.D. that his interns only laugh at his jokes to suck up to him.

Meanwhile, Carla visits Elliot at her new job and inadvertently creates an awkward new nickname for her friend when she tells her colleagues an embarrassing story. Elsewhere, Dr. Cox prevents Turk from warning Jordan about a notorious con man patient (guest star Alexander Chaplin) in hopes that she'll be mortified and quit.

J.D. struggles with decision-making and responsibility now that he is an attending physician, all the while Dr. Cox is testing his skills. And while Elliot is learning the ropes in her new hospital, she still feels more comfortable hanging out at Sacred Heart. Meanwhile, Carla is ready to have a baby, but Turk afraid of moving forward continues to slip her birth control pills.

With their residency over, there are big changes at Sacred Heart. J.D. moves out to live on his own while Turk and Carla consider a family change. Meanwhile, one of the residents receives a job offer from another hospital.

When Turk saves a man's life at the local taco stand, he tries to keep his ego in check as Dr. Cox takes all of the credit. To avoid ruining her new relationship with Jake, Elliot enlists J.D.'s help to prevent her dates from becoming too intimate. Meanwhile, Dr. Kelso terrorizes the hospital from the seat of his new Rascal motorized wheelchair until Janitor takes matters into his own hands.

When the brother of an elderly patient tries to sue his sister's well-intentioned neighbor for helping her decide what course of treatment to take, J.D. begins to question his faith in humanity. Meanwhile, Turk struggles with sharing his true feelings about his relationship with Carla when the two begin couples therapy. On the verge of giving up on men altogether, Elliot reluctantly seeks advice from Dr. Cox on how to catch the eye of the handsome hospital visitor.

Reeling after the news that J.D and Carla kissed after a night at the bar, Turk lets the two of them decided who will take responsibility and have to deal with the silent treatment indefinitely. Dr. Cox reluctantly turns to Elliot for help getting through to a young, female patient. Meanwhile, Janitor petitions Dr. Kelso for a new janitor's uniform, only to find his coworkers no longer fear him in his new threads.

At his best friend's request, J.D. agrees to take Carla out to convince her that Turk is trustworthy and that she should move back home. What starts out as a friendly drink takes a turn into unknown territory when J.D and Carla take things a little too far and share a "friendly" kiss, something they both decide that Turk does not need to know about. At the hospital, Elliot and Dr. Kelso struggle to maintain their professionalism when faced with a young patient who is suffering from complications after experimenting with erectile dysfunction pills. Meanwhile, Dr. Cox tries to find the balance between being an over-protective father while still trying to be the cool dad after his son falls off the jungle gym.

When Carla and Turk are experiencing marital difficulties, JD and Elliot do their part to try and cheer up their friends. Meanwhile, Dr. Cox challenges Dr. Kelso to lead by example and take on a few patients of his own after Kelso criticizes his bedside manner -- but nothing works out quite as planned.

After a month of dating, J.D. is a bit frustrated that Kylie still doesn't want him to spend the night. That frustration leads to temptation when kooky psychiatrist Dr. Molly Clock returns to Sacred Heart to check on a patient. While J.D. struggles with whether or not to act on his residual feelings for Molly, Carla tries to hold back her true feelings when she learns that Turk is still talking to his ex-girlfriend. Meanwhile, Dr. Cox and the Janitor enter into a friendly wager over whether or not the Janitor can get Elliot to go out with him.

Turk and Carla ask J.D. to move out to give them some space to rekindle the romance in their relationship. Suddenly homeless, J.D. tries to use the situation to his advantage by asking his girlfriend Kylie if he can stay at her place. Meanwhile, Dr. Cox gets a visit from his high school friend Ron. Dr. Cox and Ron's relationship has always been highly competitive, and just when Dr. Cox thinks he has the upper hand, the discovery that Ron's son may be autistic sends them into unfamiliar territory.

J.D.'s imagination is sparked by a patient, a sitcom writer, who is about to receive some very bad news. J.D. ponders how the lives of everyone at Sacred Heart, including his patient, would turn out if left in the hands of the writer. While the hospital is in lock-down, the rest of the staff recounts various first date stories leading to the revelation that Dr. Cox and Carla dated in the past. Tara Reid guest stars as J.D.'s ex-girlfriend.

When J.D. and Turk spend a boy's night out clubbing, J.D. falls for a bartender who seems fascinated by his profession and asks if she can stop by the hospital. Unfortunately, she shows up with her boyfriend who has a secret that J.D. struggles to keep in order to win his new crush over. Meanwhile, Dr. Cox anxiously awaits the Lakers/Heat game but must tape it to watch after his shift. His patience is tested when the Janitor threatens to reveal the final score.

Billy Callahan is a life-loving Irishman who comes to Sacred Heart Hospital after being injured in a bar fight and teaches Turk and J.D. a lesson about living life to its fullest. Meanwhile, Carla and Elliot attempt to make social plans with each other but can't seem to stick with them, which creates tension between the two friends. Elsewhere, Dr. Cox gets a vasectomy behind Jordan's back and is stunned by her reaction when she discovers the truth.

J.D.'s ego gets in the way of his learning when Elliot, his Co-Chief Resident, continually outperforms him with their patients. Meanwhile, Turk makes light of his diabetes by inventing a guessing game of his insulin levels with the nursing staff, leaving Carla flustered. Elsewhere, the Janitor feels rejected by his secret crush Elliot when she runs off quickly from their "coffee date." When J.D. must address a bunch of pre-med students about what it's like to be a doctor, he strays off the beaten path and chooses to discuss his best moments as a doctor instead. When he challenges the rest of the staff to do so, they all reminisce about their fondest memories in the medical field. Meanwhile, Elliot is forced to face her fear of children when she must befriend a ten-year-old boy, whose father is being treated in the I.C.U. Elsewhere, Dr. Kelso gets a dose of Christmas spirit and stuns the staff with his change of heart.

When one of J.D.'s favorite patients Harrison Marks is in dire need of a kidney transplant, J.D. takes matters into his own hands by finding his long-lost son Murray in hopes that he'll donate his kidney. Despite Dr. Cox's warning to J.D. interfere with family business, J.D. persists and complications arise. Meanwhile, Elliot is fed-up with co-workers not taking her seriously, and discovers she can get her way by flaunting her femininity with some help from Carla and Jordan.

J.D. stirs things up with his newfound relationship with Neena the ice queen, who treats him with no regard and threatens everyone around her with her cold-hearted ways. Meanwhile, the battle in court continues between Mr. Korman, Neena, and Turk over the allegations that Turk destroyed Mr. Korman's tennis serve as a result of the shoulder surgery Turk performed on him.

Trouble abounds for J.D. and Dr. Cox when they feel threatened by a patients' daughter, Neena Broderick, a malpractice attorney with seemingly evil intentions. J.D. finds great difficulty in escaping her flirty and conniving ways. Meanwhile, Elliot attempts to hide Doug from Neena due to his tendency to cause medical malpractices. Elsewhere, Turk pulls a fast prank on the Janitor and doesn't like the payback he receives from him and Mr. Korman, a former patient who tries to get retaliation.

Molly announces to everyone that she's moving away for a new job opportunity, which stirs the waters of the triangle between herself, J.D. and Elliot. When J.D. and Molly end up making-out at her going-away party, Elliot plays games with J.D. when he asks for her blessing for him to sleep with Molly before she leaves town. Meanwhile, Dr. Cox has to fulfill community service hours and gets stuck riding around with a neurotic ambulance driver who drives him insane.

J.D. decides he won't start dating again until Elliot does, but doesn't expect the person she starts dating to be his brother Dan, who's still sticking around after the recent death of their father. When J.D. catches them sleeping together in the on-call room, things get a little awkward between the three. Meanwhile, after Turk is diagnosed with Type II Diabetes, he milks every ounce of sympathy out of his friends. Elsewhere, Dr. Cox and Dr. Kelso get fed-up with Molly's perception that everyone is good at heart and seek to prove her wrong.

J.D. dismayed when his brother Dan shows up with news of their father's recent death. Everyone faces the challenge of how to be sensitive to J.D. and stumbles over their words of condolences. Meanwhile, Turk has symptoms of diabetes but in his stubbornness refuses to admit it or visit a doctor as Carla wishes. Molly must employ some advanced psychology to force him to face the issue.

When J.D. decides to use a tape recorder to gather his thoughts, the voice-overs transfer to Elliot, who makes new friends with Molly and gets the courage to tell-off her foil Dr. Cox. However, Eliot soon realizes that Molly is not quite the person she thought she was when her felon boyfriend is admitted to the hospital after stealing a car and crashing it into a telephone pole. Meanwhile, in the world of the newlyweds, Turk is constantly waking up Carla when he goes to bed; and J.D. has concerns that Dr. Cox may have ruined his credibility with the new residents.

When J.D. asks Dr. Cox to give the residents a pep talk, Dr. Cox takes things a little too far by telling them that inevitably, they will eventually kill a patient or two, which throws the staff into a frenzy. Meanwhile, Carla becomes so annoyed with Turk's stuffed dog Rowdy, she takes drastic measures to get rid of him. Elsewhere, Elliot fights for a patient to receive a heart valve, and becomes frustrated when Molly steps in to thwart her efforts.

J.D. has become chief resident and is flaunting his new title and pay check all over the hospital. J.D.'s new sense of pride seems to be especially hurtful to Turk, who is still a resident and currently being denied more difficult surgeries by his new attending physician. Turk's ego is further hurt when J.D. doubts his abilities as a surgeon. J.D. and Elliot's relationship has taken a turn for the worse when J.D. is accidentally called the co-Chief Resident and Elliot the Chief Resident. This mix-up lifts Elliot's post break-up spirits. Dr. Cox and Jordan are having relationship problems as well when they find out that their divorce was not final. Dr. Cox's other annoying woman in his life, Dr. Molly Clark, faces off with him over a plastic surgery patient.

Just when J.D. thinks he's landed the chief resident position, Dr. Molly Clock convinces Elliot that she should apply as well. Carla becomes green with envy as everyone starts listening to Dr. Clock's advice, leaving Carla feeling like she's losing control. Meanwhile, Dr. Cox and Turk, find great difficulty in the removal of a light bulb from a patient's posterior.

It's J.D.'s last week as a resident as he tries to smooth over his relationship with Elliot after their break-up. The former couple squabble over having to share time with newlyweds Turk and Carla, who have their own issues when Carla starts to change everything about her new hubby. Professional yet quirky new psychiatrist Dr. Molly Clock starts her first day at Sacred Heart turning heads and forging new relationships with the staff. She befriends a lonely Elliot and tries her best to help J.D. and Dr. Cox with a patient offering profound and helpful insight. Meanwhile, when Turk's car blows up, Carla lets him pick out the new ride only to find that instead of a car, he buys a Vespa scooter further proving Carla's point that she needs to always be in control.

The day of the wedding Turk picks up an extra shift at the hospital to get some extra time off for his honeymoon. Inevitably, what should be a very simple procedure turns into a life and death situation when Turk discovers complications with his patient and he gets stuck at the hospital. Meanwhile, a very stressed out Carla tries to manage the situation at the church while her groom is missing, her brother Mark continues to antagonize her, and her guests are becoming unnerving. After telling Elliot he is not in love with her, JD tries to put together the broken pieces of their friendship by getting Sean back together with Elliot.

The eve of Carla and Turk's wedding gets off to a rocky start when Turk forgets to pick up Carla's brother Mark from the airport. The mounting pressure does nothing to help Turk as he finds himself at a loss for words while writing his own wedding vows. Another relationship seems to be in trouble as well when J.D realizes that he does not love Elliot and goes to Danni for advice on how to tell her. Meanwhile, Dr. Cox is going through fighting withdrawals when Jordan refuses to fight with him anymore and the hospital staff pays the price. Ken Jenkins and Neil Flynn also star

J.D. and Danni breakup again because of J.D.'s continuing feelings for Elliot. When Elliot asks Sean to move in with her, J.D. decides he has to take action. Elsewhere, Dr. Kelso and Dr. Cox face-off over whether or not the hospital should provide full body scans. As the wedding grows nearer, the pressure to make their special day perfect starts to get the best of Carla and Turk.

After an unexpected night of passion with Elliot, J.D is left frozen in shock when Sean comes back into Elliot's life. Carla and Turk try to pass on advice to J.D but despite their efforts he finds himself in bed with a reinvented Danni on the rebound. Dr. Cox is shocked by the realization that his hatred of Dr. Miller may actually be an attempt to mask a secret crush. Meanwhile, Dr. Kelso and the Janitor square off on the do's and don'ts of hospital security.

J.D is left bored after he clears his schedule to help Dr. Cox with a special project only to be pushed aside for a new intern. With some free time on his hands JD offer to help Elliot with Kids Day when she discovered she couldn't attend. His good deed does not go unnoticed, even though Sean shows up unexpectedly. Meanwhile, Turk begins to get nervous about the wedding when he has to send out the invitations. Dr. Cox and Dr. Kelso end up fighting for the attention of the new female doctor, Dr. Miller.

The return of Dr. Ron Ramirez to Sacred Heart from his philanthropic work abroad, sparks some doubts in Carla's mind about her feelings for Turk. When she accepts a dinner date with him, J.D. is torn between telling Turk the truth and keeping his promise to Carla. Back at the hospital, Elliot takes a stand for her patient despite the fact that Dr. Cox believes he is a drug addict faking abdominal pain to get medication. Meanwhile, the Janitor introduces J.D. and Turk to his "twin" brother, but they aren't buying it.

In a unique episode, the path of a butterfly is the deciding factor in the fate of the young doctors. At the beginning, a butterfly landing on a woman's breast sets off a chain of events ultimately resulting in the death of J.D. and Turk's patient, leaving them to wonder what they could have done to change the outcome of their day. Taking a step back in time, the butterfly takes a slightly different path, landing instead on a man's chest setting in motion an alternate chain of events that leads our characters down a very different path. Will this be enough to change the fate of their patients?

Jordan's friends Allison and Maddie come to town to provide moral support for Jordan after her brother's death -- however, their constant presence does not provide any relief to Dr. Cox. Meanwhile, Turk tries to impress the new female surgeon, Dr. Miller, but his first encounter serves only to leave him baffled by the opposite sex yet again. Attempting to reinforce the policy on sexual harassment at Sacred Heart, Carla is forced to teach a course where Dr. Kelso is the number-one student.

Danni and Jordan's brother, Ben returns from traveling the world, a trip he's been on since his leukemia went into remission. When Dr. Cox learns he hasn't seen a doctor since he left, he recruits J.D to do a thorough work up. Unfortunately, forcing J.D. to drop his other patients to run tests on Ben is a decision Dr. Cox soon regrets. Meanwhile, Elliot's concern about a bunion on her foot leads to the revelation that Carla has been hiding something from Turk.

Elliot finds herself perplexed over a simple procedure that leaves her questioning her career path. She goes to seek Dr. Kevin Casey's advice but becomes frustrated when she learns that the epiphany toilet, which the Janitor secretly installed on the roof, is getting more attention than she is. Meanwhile, Turk asks JD to be his best man at his wedding, which sends JD into sheer bliss momentarily until he hears a phone message that makes him question his friendship with Turk.

Dr. Kevin Casey comes to Sacred Heart as a visiting medical doctor and an accomplished surgeon despite being diagnosed with a severe case of obsessive compulsive disorder. Dr. Casey touches everyone at Sacred Heart in a special way during his visit, beating his old resident buddy Dr. Cox at his own game of wits, leaving Turk in the dust with his surgery time and teaching J.D some very valuable lessons about learning to believe in himself. Meanwhile, Dr. Kelso strikes a deal with the Janitor to take care of the hospital's garbage, only to find himself eyeballs deep in it.

J.D. decides to break it off with Danni because things are moving too fast, but he dreads the idea of having to do the deed. Ultimately, he is spared when Danni beats him to the punch by having Jordan break up with him for her. Meanwhile, Dr. Cox discovers that parenting has unearthed his sensitive side and as a result, the residents don't fear him the way the used to, a situation Dr. Cox is more than happy to remedy. Elliot struggles to convince Dr. Kelso and her colleagues that her new look does not make her less of a professional.

Dr. Cox' disdain for Turk reaches a new high when Turk refuses to perform a liver transplant on Cox' patient after he admits to breaking the rules regarding the waiting list. Meanwhile Carla and Elliot try to make a dying patient's wish to lose her virginity come true by finding her a man -- a request that proves to be more difficult than they imagined. While the others are fighting for their patients, J.D. is blind-sided by Danni's suggestion to move in with him and he struggles to slow down their new relationship.

Elliot's discomfort with sex and the proper terms for the human sexual organs reaches a new high after she performs a pelvic exam on an elderly woman with surprising results. J.D.'s patient, Mr. Cantwell refuses to have surgery to prostate cancer surgery because he doesn't want to risk his sexual relationship with his wife. Meanwhile, Dr. Cox can't find the time to have a sexual relationship with Jordan and Carla surprises Turk with the news that she's decided they should abstain from sex until their wedding - a decision she ultimately reevaluates when she needs a little comforting.

Turk joins J.D. in the winner's circle after he performs his first solo surgery brilliantly, leaving Elliot struggling to find her own defining moment. While the young residents wrestle with their professional challenges, Dr. Cox deals with the reality of growing older when his back goes out and he refuses to admit that he needs help. Meanwhile, J.D. gains some insight into the life of the Janitor that sheds new light on his intriguing history.

As J.D.'s relationship with Danni progresses, he and Dr. Cox are forced to spend more time together outside of the hospital, to Perry's great chagrin. However, their antics, designed to sabotage each other, end up only aggravating Jordan and Danni. Meanwhile, as they each deal with problems in their own relationships, Carla and Elliot work to find their own balance between their personal and professional relationships. Dr. Kelso accidentally punctures his eardrums giving the hospital staff an opportunity to tell him how they truly feel about him.

A chance meeting in the elevator sparks a new friendship between J.D. and the mysterious Danni whose ongoing dialogue about her ex forces J.D. to talk about his feelings for Elliot. Meanwhile, Carla's brother Mark comes to visit, but things are a bit uncomfortable since he still hasn't forgiven Turk for mistaking him for a waiter at their mother's funeral. Unfortunately, Turk can't even apologize because Mark refuses to speak anything but Spanish. At the hospital, Dr. Cox is relishing his new role as Residency Director, but is a bit surprised when J.D. doesn't immediately follow his lead in contradicting Dr. Kelso's orders.

Dan decides to stay with J.D. for a while, since their mom's new fiance kicked him out, but after spending some quality time with him at the hospital, Dan isn't sure if he likes the cynical person his little brother has become. Meanwhile, Elliot tries to deal with her own problems of having a long distance relationship and decides to try moonlighting as a pet examiner with Carla to save up money to go visit her boyfriend as Carla saves up for her wedding.

Sean is a bit surprised and disturbed to learn about Elliot and JD's history together, but it turns out his anger is really a cover up for the fact that his "little" work trip is actually six months long and he doesn't want Elliot to explode. J.D., who has been watching their relationship develop, finally acknowledges that he still has feelings for Elliot and that he has to come to either accept her new relationship or do something about it. Meanwhile, Dr. Cox. is forced to swallow his pride and actually ask for help when he decides to throw his hat in the ring for the Residency Director position at the hospital. Carla picks up an extra shift as a surgical nurse, which allows her to spend the day working alongside with Turk, to his great annoyance.

As third year residents JD, Turk and Elliot get their own set of interns and realize that they each have very different approaches training them. When JD needs help with training his interns he turns to Elliot and who refers him to her boyfriend, Sean who shares some of the training tips he's picked up during his time at Seaworld. At the same time, Elliot experience with her interns makes her realize that she needs to stand up to Dr. Kelso. Meanwhile, Dr. Cox and Jordan are having some trouble finding a doctor for their kid, until they meet Dr. Norris whose abrasive, yet somehow familiar personality makes his the perfect candidate for the job.

When Carla and Turk set the date for the wedding, JD realizes this is the end of an era in their friendship, but his efforts to celebrate this milestone are rebuffed by Turk. Elliot is determined to win back her old boyfriend Sean, but is again faced with tough choices between a relationship and her career.

A run in with her old flame Sean and an accident in her new car leads Elliot to the realization that nothing good has happened to her in the three years that she has been at Sacred Heart, driving her to take drastic measures, including a complete makeover, both inside and out in an attempt to change her luck. Meanwhile, JD gets stuck with a patient whose diagnosis he can't quite pin down and when Dr. Cox denies him any assistance, he looks to Carla and Turk to help him out. While Dr. Cox is busy not helping J.D., he takes the time to make amends with Dr. Kelso.

When J.D. and Turk are paid a visit by their college cohort, Spence -- who is now leading a life of luxury as an investment banker -- they are struck by the daily sacrifices they make as residents leading them to re-evaluate their career choices. Spence's visit also triggers the reveal of another dirty little secret - Dr. Cox learns the truth about his relationship to Jordan's baby, but the truth may be more than he can handle. Meanwhile, Elliot's skills and patience are tested by the unrelenting Dr. Kelso.

The unexpected death of Carla's mother causes her to second guess her wedding plans with Turk. J.D. turns to Elliot for help when his romance with Jamie begins to fizzle, and Dr. Kelso appoints Dr. Cox to teach a daily seminar on doctor-patient relations as punishment for his inappropriate behavior.

JD finds himself in a tough spot after a romp with Jamie in a funeral home leads him to an accidental sighting of the janitor's "private" equipment and a language barrier with a patient results in some major miscommunication. Meanwhile, Turk starts having steamy dreams about Elliot making things a bit uncomfortable, especially when Carla finds out and Jordan forces Dr. Cox to watch her still-to-be-named son.

J.D., begins a surgical rotation and in an attempt to lose his nerdy medical resident reputation he reveals a secret about Turk to get "in" with the rest of the surgeons. While his betrayal earns him a place at the surgical table, it may cost him a friendship. Meanwhile a miscommunication between Elliot and Paul leads to confusion about their true feelings for each other and Dr. Cox plays a "deadly" practical joke on Dr. Kelso.

When J.D. starts getting flak from his friends about dating a woman who is visiting her comatose husband, he quickly becomes defensive. Elsewhere, Carla and Turk fight over every little thing since they became engaged, Dr. Cox is jealous of all the attention Jordan gives the newborn baby and Elliot overreacts because she thinks Paul doesn't trust her.

While J.D. struggles with whether to tell Dr. Cox the truth about the baby's paternity, he gains some valuable insight into Dr. Cox' troubled mind and his relationship with Jordan when he meets Dr. Fisher, a former student of Dr. Cox. When Dr. Kelso prevents Turk from playing his pre-surgery basketball game, Turk retaliates by cutting the cord to Kelso's beloved Ms. Pac Man machine. Meanwhile, Carla worries that she's lost her sex appeal since she got engaged and Elliot's lucky streak continues as she makes a new friend in the janitor.

Elliot tries to keep up her perfect persona for her new boyfriend for fear he won't like her if she tries just being herself. At the hospital, things misfire for Jordan and Dr. Cox when a miserably executed practice run of the baby's birth turns into the real thing -- and in the throes of labor, Jordan reveals the identity of the baby's father to J.D. Meanwhile, Turk and J.D. can't handle their guilt when they believe that one of the golf balls they hit off the hospital roof caused a car accident.

At a bar, Carla makes a decision on the right answer to Turk's proposal while Elliot thinks she has struck gold when she meets Paul, a charming man whom she recognizes from the hospital and assumes is a fellow physician. However, the next day, she learns that he is a nurse, and she is forced to put her ego in check to give the relationship a chance. In the meantime, J.D. enjoys a rare day when everything goes right while Dr. Cox' world is in chaos as he manages to alienate himself from all of his colleagues at the hospital.

Dr. Kelso assigns J.D. to work on a patient with Dr. Townshend, but things go downhill when Dr. Townshend insists on using his "old school" treatment method which proves to be an alien concept to an inept J.D. While Kelso defends his friend's technique, Dr. Townshend's lack of current medical knowledge forces Kelso to admit his mistake. Meanwhile, Carla tells Turk she needs time to consider his proposal because she thinks he is still too immature, prompting Turk to invite his married brother for a visit to show Carla how seriously he is. In addition, Elliot learns the sex of Jordan's baby, but Dr. Cox and his ex-wife can't agree if they want to know.

Turk makes plans to propose to Carla, only to watch all his plans fall apart when one thing after the next prevents him from actually popping the question. Meanwhile, J.D. comforts a patient in need of a heart transplant by explaining his theory about the balance between life and death in the hospital. This theory is put to the ultimate test when a husband must choose between saving his unborn child and his wife's well-being. On another front, Elliot prevents Dr. Kelso from expanding his office space when she launches a campaign demanding that Dr. Kelso build a separate female locker room.

J.D. wrestles with his feelings for Elliot and can't handle being "just friends" especially when Elliot needs a friend the most. Meanwhile, Dr. Cox and Carla realize they are not as close as they used to be when they fail to read each other's signals while treating a hypochondriac patient.

J.D. and Elliot continue to seek relief from their problems by sleeping together, but when things start to get a little out of control, Elliot proposes that they become "sex buddies," which seems at first to be the perfect solution. Elliot thinks she has a handle on things, but when Turk is forced to take the heat from Dr. Cox for her mistakes with one of their patients, he tries to lighten her load by suggesting that her patients switch to the care of other doctors. His good intentions put him at odds with both his friends and they are forced to reevaluate their "arrangement." Meanwhile, Dr. Kelso accuses Carla of making her relationships with her patients the priority over their well-being.

Elliot continues her search for a place to live, JD asks the gift-shop girl out on a date, and Carla has romance problems with Turk. Meanwhile, Dr Cox lets newly pregnant Jordan move into his apartment.

JD is riding high after correcting a misdiagnosis by Dr. Cox, but is jerked back to reality when he is unable to explain the death of his next patient. Meanwhile Elliot is struggling personally and professionally as she is forced to relinquish all of the luxuries that her father had been financing -- including her apartment -- and a malpractice suit is brought against her by a patient for saving his life. Carla learns that sometimes it is better to keep her opinions to herself when the fallout of a conversation with Dr. Cox about his relationship with his ex-wife lands Jordan in the hospital.

Dr. Cox's relationship with pharmaceutical rep Julie Keaton is off to a good start when they wake up together, but when Jordan shows up with a surprise, Cox if forced to choose between the new woman in his life and the ex-wife he can't get out of his life. Elsewhere, J.D. and Turk take advantage of the hospital's supply closet, while taking on extra shifts at a free clinic to supplement their income as residents and Carla encourages Elliot to stand up to her father when he pressures her to pick a specialty more "suitable for a woman," especially when it will prove Dr. Kelso wrong.

Dr. Cox's efforts to take a stand against the evil drug companies are thwarted by the seductive charm of pharmaceutical sales rep Julie Keaton. She takes advantage of the fact that Dr. Kelso has assigned Dr. Cox to coordinate a dinner reception sponsored by the drug company to pursue Dr. Cox in other arenas. Elliot and J.D. disagree about who is the better doctor, but when Elliot disregards his advice about how to treat her patient and the patient dies, it is actually J.D. that learns a lesson about taking chances. Meanwhile, Turk tries to encourage Carla to take her career to the next level by enrolling her in a nurse practitioner course.

J.D. gets a surprise visit from his ne'er-do-well brother Dan, and becomes jealous when he and Elliot hit it off. Meanwhile, Turk feels guilty after betting Dr. Cox $20 that surgery would save a patient's life.

J.D. gets a reality-check when the white lab coat that he is allowed to wear as a resident does not bring the new level of respect he thought it would, and Elliot earns a reputation after her first one-night stand with another doctor. Meanwhile, Turk tries to fit-in with the surgical "boys club" and finds himself struggling with whether being part of the club is worth the price of admission. Dr. Cox teaches Carla a lesson about surviving in medicine, by enrolling a patient into a clinical trial without approval.

Elliot's latest try to win points with Dr. Cox has her delivering bad news to patients on Dr. Cox behalf, she thinks it's fine at first, but she soon learns that also she has a limit. J.D. betrays Carla by telling Turk some of Carla's feelings and insecurities towards him, while Turk has been selected to represent Sacred Heart on a trip for the "Doctors Without Borders" program, but when he finds out that his toughest rival Bonnie wasn't chosen just because she is female he isn't so sure he wants to go.

To motivate the residents Dr. Kelso announces that the person with the most interesting case study will get to go to a medical conference in Reno which makes all the residents look like crazy to find themselves an interesting case, everyone except J.D.. Elliot and Carla realize that they have more in common than just their relationships with J.D. and Turk, meanwhile Turk is in big trouble as he forgot to ask for a new laser for the surgical team on the one day that Dr. Kelso grants requests.

It's JD's, Turk's and Eliot's first night all alone and without supervision at the hospital. The night they have all been waiting for, but they soon find out that it isn't as easy as they have imagined it when they are flooded with work which forces them to seek help from Carla, and even the Janitor. Meanwhile, Dr. Cox has agreed to introduce Dr. Kelso at an awards ceremony which he is not looking forward too at all.

After Jordan dropped the bomb that JD and her had slept together, Turk knew that Dr. Cox still likes Carla and the fact that Elliot has feelings for J.D. things are a bit tense at Sacred Heart. Turk and Carla try to deal with the fact that they have trouble trusting each other and J.D. tries to fix both his relationship with Dr. Cox and Elliot, which turn out the be much more difficult than he thought.

As J.D., Elliot and Turk end their first year as interns, they try to help an elderly man get an operation despite a lack of insurance, which draws the ire of Dr. Kelso in the season finale. Meanwhile, when Dr. Cox and his ex-wife Jordan's relationship reaches a breaking point, she drops a bombshell on the gang.

J.D. begins to think Dr. Cox may have feet of clay when his mentor seems unable to deal with his friend Ben's potentially fatal illness. Meanwhile, Turk is upset at the news that he isn't the best surgical intern on the team, but he's even more frustrated when he finds out who is. Dr. Kelso asks Elliot to write a supposedly anonymous evaluation of him -- but the joke's on Carla when she volunteers to ghost-write.

J.D. sees the human side of his mentor when he has to deliver disturbing test results to Dr. Cox' good friend and former brother-in-law, Ben Sullivan. Meanwhile, Elliot and Turk are both embarrassed by patient mix-ups.

When J.D. accidentally gets stuck with a needle that may have been infected with hepatitis, he starts to shy away from dealing with his sicker patients. Meanwhile, he and Elliot are forced to deal with four doctors who become ill while attending a local medical convention. Elsewhere, Turk pays a high price when he goes to Dr. Cox to help him get in shape.

J.D. gets a lesson in competitiveness when he asks for Turk's support in persuading his patient to opt for medical treatment over surgery, and Elliot develops a crush on one of her patients, but her own fumbling, insecurity and bad timing are compounded by their similar traits. Meanwhile, Dr. Kelso finds Dr. Cox's Achilles heel when he fires the nurse who makes Cox's favorite cup of coffee.

While Elliott and Turk's parents pay a visit to see them present at a medical conference, J.D.'s father Sam makes a surprise visit, leaving all three interns in a state of emotional distress as old parental behaviors come into play. Meanwhile, Carla gets the biggest shock of them all when she discovers she is exactly like Turk's mother. John C. McGinley and Ken Jenkins also star.

J.D. suffers from guilt-induced insomnia after Carla points out that he avoided a terminally ill patient because the man was a difficult pill. Meanwhile, Dr. Cox' ex-wife Jordan interferes in his new relationship with medical student Kristin and Turk and Elliot obsess over evidence that Dr. Kelso might be concealing a musical past.

A gleeful J.D. can't wait to get his hands on a goofy new medical student so that he can make his life as miserable as he was early on -- but his harsh words have an unintended effect on the mistake-prone young man. Meanwhile, another medical student draws Dr. Cox' appreciative eye and Elliot is paired with the son of the hospital's chief executive officer.

When a rampaging Dr. Cox suddenly shows up at his door in a strange funk, J.D. sees a possibility of a breakthrough in their relationship -- until he learns that Cox loses control in almost an annual rite. Meanwhile, Turk elects to partner with Elliot on a research project so that he can maneuver her back with his buddy J.D., and Carla struggles to fulfill a comatose patient's final request -- playing heavy metal music in his room.

J.D. and Elliot struggle to keep their newfound sizzling romantic relationship under wraps while at the hospital, but when the inevitable happens, they boldly display their mutual affection in public -- which only results in turbulence when she thinks he doesn't respect her enough. Meanwhile, Dr. Kelso orders the entire staff to participate in a psychological study about their innermost personal and professional feelings.

J.D. strikes up a romantic relationship with an attractive social worker, but he soon learns that Elliot is not only jealous but thinks his new squeeze is bad news. Meanwhile, Turk discovers that Dr. Cox also has romantic designs on Carla while Carla encounters hard stares from her fellow nurses when she strikes up a car-pool friendship with the hated Dr. Kelso.

J.D. is frustrated when his budding romance with an ex-patient wilts as their dates are constantly interrupted by emergency calls while the beastly Dr. Kelso becomes less fearsome to the wide-eyed interns when Dr. Cox informs them how to avoid the blowhard's bite. In addition, a perplexed Carla and Turk deal with their first sexual intimacy complication.

A hyped Dr. Cox challenges the hospital staff not to lose a single patient before his shift ends at midnight while J.D. is enchanted by an unseen female patient who is trapped inside an MRI machine -- and even considers asking her out. Elsewhere, Turk and Carla grapple with a pained patient as they try to re-set his dislocated shoulder.

As Christmas nears, Turk's idealized notion of the wonderful holiday is challenged by the grim reality of the hospital while Dr. Cox asks J.D. to videotape the birth of his friends' baby -- with calamitous results. Elsewhere, Dr. Cox also has to contend with his abrasive ex-wife Jordan, and Elliot searches for a sickly teenaged woman who disappeared after Elliot told her she was pregnant.

J.D. gains a new nickname -- "Scooter" -- and his relationship with Nurse Carla evolves as he begins to out-distance her medical knowledge while Turk trembles when he learns that his secret lunch spot was claimed years ago by the territorial Dr. Kelso. Elsewhere, Dr. Cox gives Elliot the evil eye when she won't discharge a troublesome patient.

As Thanksgiving nears, J.D. is stricken with appendicitis and gains a new and horrifying perspective on what it means to be a patient, and to make matters worse, it's Turk who will be doing the surgery. Elsewhere, Elliot resents the fact that J.D.'s patients prefer his care to her cold hands when she has to cover his cases for him, and Dr. Cox makes sure that his former medical mentor gets proper care from the nasty Dr. Kelso.

J.D. and Turk become reluctant heroes when they sneak off to a strip bar for lunch and instead save the life of a protester in front of TV cameras -- and the crafty Dr. Kelso wastes no time in exploiting their "salt-and-pepper" images in an ad campaign to tout the hospital's diversity. Elsewhere, J.D. risks Dr. Cox' considerable wrath when he insists that his mentor complete his evaluation form while Elliot feels slighted by Carla.

When everything is going great for J.D., he aces rounds, and is respected by the other interns, a big rival appears, a fellow intern Nick who promises to challenge J.D. to become the doctors favorite intern. Turk looses his confidence after he may have made a mistake but does not tell Carla about it. Meanwhile Dr. Cox expresses a romantic interest in Carla.

Elliot's patient, a psychiatrist whose jaw has been wired shut is put to the test when he has to listen to Elliot ramble and cannot do a thing about it, meanwhile J.D. has a very important patient, a hospital board member by the name of Jordan Sullivan. Dr. Cox future is quite unclear as his fate will be decided by the hospital board. Turk and Carla's relationship is put to the test when Carla blames Turk for an injury her mother sustains.

J.D. is torn when he discovers that Dr. Cox has falsely appropriated the medical insurance of a deceased patient in order to help another more needy patient, but when he inadvertently informs Cox' rival the two veteran doctors battle for J.D.'s soul during a good-vs.-evil game of golf. Meanwhile, Turk frets that his girlfriend Carla might figure out where he got his new gift for her. Meanwhile Elliot discovers exposing herself heals patients.

J.D. treats an old lady whose kidneys are failing but gets too attached to her, and when she refuses to undergo dialysis it is quite clear what will happen. While operation on a very nice hernia patient Turk gets a big chock. Elliot and Carla are not on the best terms with each other but has to work together to save the life of a patient.

As Turk now has a girlfriend he has been spending less and less time with his best friend J.D. which has made J.D. feel left out and abandoned, to add to that J.D. suspects that Turk botched a surgery and is afraid that he himself can get into trouble for not reporting it. Elliot tries to get some help from Dr. Cox, but he is far from helpful as he does not want to be the interns' designated mentor.

After one of J.D.'s patients recover from cancer he tried to get the patient to stop chain smoking, which proves to be more difficult than he thinks. Elliot becomes public enemy number-one to the nurses because of her unsociable bedside manner and general attitude. Turk persistently tries to ask Carla out again and again only to get denied.

The first day for the new interns at Sacred Heart is today, which means that J.D., Turk and Elliot now have to start practicing what they have learned in medical shool. They soon realise being a doctor was not really how they imagined it...
