Although the show is generally comedic, it regularly incorporates touching scenes and drama alongside its more humorous and ridiculous elements. The half-hour episodes are usually internally narrated at the start and finish by the main character J.D., with other characters' insights throughout. Each episode is usually self-contained and covers a theme in the professional and personal lives of J.D. and others at the hospital, though story arcs sometime stretch over several episodes or even seasons. The series is also known for J.D.'s recurring bizarre day-dream cut-away sequences and its unique point of view as a telling of J.D.'s personal experiences.
The series features a large cast of regulars including doctor Elliot Reed (Sarah Chalke), with whom J.D. shares an on-again off-again relationship throughout the series, as well as J.D.'s best friend, the surgeon Christopher Turk (Donald Faison), with whom he is very close. Turk's relationship and eventual marriage to head nurse Carla (Judy Reyes) comprises a major storyline in the show. J.D. spends a lot of time trying to earn the approval of his cynical but secretly good-hearted mentor, doctor Perry Cox (John C. McGinley). Throughout his tenure, J.D. is embroiled in a strange feud with a character known only as The Janitor (Neil Flynn) that begins on J.D.'s first day at the hospital. The staff at Sacred Heart are often at odds with its Chief of Medicine, Bob Kelso (Ken Jenkins), who is usually more concerned with hospital revenue then providing care. With the help of his friends, J.D. maneuvers through life, love, and fights to do right by his patients no matter the cost.
