In the backgrop city of Seattle, high school student Matt Leland falls for new girl Casey Roberts, originally from Chicago. Matt and Casey start to have a sexual relationship. Casey's parents disapprove and try to stop the teens from seeing each other. After Casey deliberately activates the school's fire alarm, without a fire actually threatening the school. This gets her suspended, during which time Casey argues with her parents who plan to send her to a boarding school. After overdosing in rebellion, her parents have Casey committed. Matt slips in and helps Casey escape from her room at the psychiatric ward, running away and dealing with Casey's major personality disorder. This disorder causes Casey to be quite eccentric. Both her devil-may-care attitude and her impulsiveness are seen as evidence of her illness, which also shows her experiencing extremes in mania and depression. Between her recurring and intense emotional bouts, feelings for Matt, and her fear and destruction, her personality is ruled by the illness. During the pair's time together, Matt constantly puts her needs before his own, a gap which widens further and further as Casey's mental state deteriorates. Eventually the kids head south to Mexico in Matt's SUV, where Casey's recklessness and emotional nature continue to spike upward. The SUV crashes and the teens abandon it, resolved to hitchhike the rest of the way. A salesman, played by Liev Shreiber, gives them a ride. However, after placing his hand on her crotch and Casey presses a lit cigarettes to hit face, the salesman throws Casey out and picks a fight with Matt. Matt and Casey wind up stealing the salesman's car to continue the trip down to Mexico. Casey becomes even more worried with Matt doing his best to help her out. Eventually Casey threatens to kill herself, later threatening to kill Matt using a gun she found in the salesman's car. Matt is fearful for Casey's sanity at this point and manages to convince her that she really does need help. She believes him and they head back to Seattle, with Casey being readmitted to the psychiatric ward. Some time later, Matt receives a letter from Casey where she explains she's moved back to Chicago and has fond memories of their time together.
