One of the main storylines in this episode centers around Sister Mary Cynthia, who is facing struggles of her own as she tries to come to terms with the traumatic events she experienced in South Africa. When she begins to struggle with her mental health, her fellow nuns and the midwives rally around her to provide her with the support and care she needs.
Meanwhile, Nurse Crane becomes involved in a case that highlights the dangers of back-alley abortions, as she tries to help a young woman who has attempted to terminate her own pregnancy. Another case that arises in the episode involves a young mother who is struggling with postpartum depression and anxiety. As the midwives work to support her and her family, they also come to recognize the importance of mental health care for mothers, both during and after pregnancy.
Throughout all of these challenges, the midwives and nuns must also navigate their own personal struggles and relationships.
