Dr. Alex Reid is a surgical resident at Hope-Zion Hospital who is engaged to the hospital's Chief of Surgery, Dr. Charles Harris. On the day of their wedding, Alex and Charlie are involved in a car accident that leaves Charlie in a coma. Alex is devastated and struggles to come to terms with the fact that her fiancé may never regain consciousness.
As Alex begins her first day back at work, Hope-Zion's ER is inundated with patients after a bus accident. Among the victims is a young man named Joel Gideon who is brought in with a severe head injury. Despite Alex's best efforts, Joel's condition continues to deteriorate.
Meanwhile, Dr. Harris, trapped in a comatose state, finds himself wandering the halls of Hope-Zion as a ghost-like figure. Charlie is able to communicate with other coma patients, including a young woman named Dana who urges him to fight to stay alive.
As the day progresses, Alex receives a shocking diagnosis of her own, further complicating her emotional state. She turns to her fellow doctors, including the charming Dr. Joel Goran and the confident Dr. Maggie Lin, for support.
