Dr. Alex Reid serves as the Chief Surgical Resident at Hope Zion Hospital and she is facing one of her toughest cases yet in season 2 episode 11 of Saving Hope entitled "En Bloc."
Resident Iris Kim, a surgical resident under Alex's guidance, faces a troubling case of a young woman who has recently discovered she has a tumor in her spine that will result in paralysis if not removed. The patient is busy with preparations for her wedding, but the surgery would mean placing the bride-to-be in an induced coma, halting any future wedding planning.
As this case begins, the doctors at Hope Zion Hospital are prepping for a conjoined twin separation surgery, leading Dr. Shahir Hamza, the Head of Neurosurgery, to decide which of his protégés receives the responsibility of overseeing the operation. Dr. Dana Kinney and Iris in particular have made a strong impression on the Head of Neurosurgery with their technical abilities.
However, Alex receives a shocking surprise when she discovers that her ex-boyfriend Charlie Harris, the Chief of Surgery, is not dead after all. Instead, he is in a coma after a car accident and has been communicating with spirits in the hospital ever since.
As Charlie lingers in a coma, he finds himself stuck in a limbo-like state where he is constantly communicating with spirits and learning more about his own life and those around him. However, he is struggling to find a way back to his physical body as the hospital staff works to keep him alive.
Meanwhile, the team prepares for the two surgeries as they simultaneously deal with their personal dramas. Kinney has to cope with her pain after her divorce and the loss of her father, and Shahir must navigate his faith as he worries about the ramifications of working in a hospital as a Muslim.
Between the technical challenges of both surgeries and the characters' intense emotional states, the episode explores the themes of loss, medical ethics, resilience, and the acceptance of change as the show reaches its climactic midpoint.
