JD, Turk, and Elliot are doing the rounds, and they come across Mrs. Guerrero, who's a cleaner at Sacred Heart. She's been working there for over 20 years, and she's been a mother figure to everyone in the hospital. However, she's admitted to the hospital after a heart attack. Perry Cox asks JD and Turk to find out what happened to her and to teach her daughter, who's deaf and dumb, how to manage her medications.

Elsewhere in the episode, the Janitor teaches Turk about karma, and Dr. Kelso asks JD for a favor to keep his job as the Chief of Medicine.

The episode opens with JD explaining that as doctors, they're supposed to be detached from their patients, but sometimes they can't help but get emotionally invested. This episode explores that theme as they all try to help Mrs. Guerrero.

JD and Turk try to find out what caused Mrs. Guerrero's heart attack, and they discover that she was under a lot of stress due to a family issue. They do their best to comfort her and to teach her daughter how to manage her medications.

The episode ends with the staff of Sacred Heart coming together to show their appreciation for Mrs. Guerrero. They throw her a surprise party, and even The Todd delivers a heartfelt speech. Mrs. Guerrero is overcome with emotion, and she thanks everyone for their kindness.
