The third kingpin featured in this episode is a Russian immigrant known as "The Doctor." He is the head of a prescription drug trafficking ring that supplies powerful painkillers and other prescription drugs to people across the city. The documentary shows how The Doctor obtains the drugs by exploiting the vulnerabilities of the healthcare system, such as using fraudulent prescriptions or by stealing them from pharmacies.

Throughout the episode, viewers get a glimpse of the devastating impact these drug lords have on society. Interviews with drug users provide insight into the dangers of addiction and the desperation that drives it.
