She goes to Burke to ask for help, but he resents her for her past actions. Eventually, he reveals that he has a trembling hand after the shooting, and the conversation transforms their relationship.

Meredith Grey struggles with her absent mother and her past relationship with Derek Shepherd. She has flashbacks of their steamy history. The two deal with the fallout from their affair, but they both constantly regal in their love for each other. Since Derek is appointed as the chief of surgery, it means he has to make rounds. Meredith struggles to watch Derek take control of the hospital, but she ultimately supports him.

The episode's only case involves two sisters who both require an operation from the surgical team. The younger of the two girls is ambitious, career-minded, and ignores her sister's concerns. They bicker and argue over insignificant things in the hospital. The older sister grows tired of it and finally speaks out, revealing the underlying fear of losing her sister. Ultimately, the sisters come to a mutual understanding, proving that family is the most important thing in times of crisis.

The episode concludes with Webber, who realizes that the patients should always come first. He finally tells Bailey to release a patient facing an emergency to another surgeon who can save his life. Bailey is shocked, but Webber is confident in his decision.
