While attending to their usual duties, the midwives encounter a multitude of different situations. From delivering twins in a prefab hut to caring for an elderly woman in her final days, each patient’s story is imbued with the warmth and compassion of these dedicated medical professionals. The hardship and difficulties faced by these women are sensitively portrayed, and their bravery and strength is acknowledged and celebrated.

The Christmas special also sees the arrival of a new nurse, Jane Sutton. Soft-spoken and graceful, she also brings with her an air of mystery. Throughout the course of the episode, the other midwives try to piece together the details of her private life. They eventually discover that she has experienced a loss that has left her adrift and unsure of her future. The other midwives rally around her, offering their support and advice as she tries to find her path forward.

A snowball fight between the nuns and the midwives erupts in the snowy courtyard of Nonnatus House, bringing laughter and joy to all involved. The highlight of the Christmas special, of course, is the community concert. The midwives and nurses sing carols and perform skits, much to the delight of the crowd. They are joined by the residents of Poplar, who form a makeshift choir and harmonize with the professionals.
