Next, we see the Nohara family at a restaurant, where they order various dishes. Mitzi is disappointed with the food and sends it back to the kitchen, which causes a heated argument with the restaurant owner. In the midst of the altercation, Shin chan accidentally drops a glass of water on himself, resulting in a minor burn on his chest. The restaurant owner insists on paying for the medical bills, and the family is led to the hospital once again.

At the hospital, the Nohara family encounters a series of mishaps, including long waiting times and mistaken identities. Shin chan's father, Hiroshi, is mistaken for a patient and nearly undergoes surgery before the mistake is discovered. Meanwhile, Mitzi is outraged at the hospital's billing system, which seems excessively high. She argues with the hospital staff, accusing them of overcharging and unethical practices, but to no avail.

As the family members wait for their turn to see the doctor, they encounter various patients with different ailments and quirks. Shin chan befriends a little girl with an ear infection, while his father gets into a heated argument with a man over a television remote. The family members seem to grow increasingly frustrated and anxious as they wait for their turn to be treated.

The episode ends with the Nohara family finally being seen by a doctor, who diagnoses Shin chan's burn to be mild and treats it promptly. The family then leaves the hospital, relieved to be done with their medical adventures.

"At Least They Pay for the Ouchie" is a humorous take on the challenges of navigating the healthcare system. Through Shin chan's family's experiences, we see the various flaws in the system, including long waiting times, high costs, and disorganized billing processes. Despite the difficulties, the Nohara family manages to find humor in their situation and come out of it relatively unscathed. The episode highlights the importance of patience and persistence when dealing with the healthcare system, as well as the value of finding laughter in unexpected situations.
