Ep 1. After The Fireworks

In the season 4 premiere of Royal Pains titled "After The Fireworks," we find Hank and Evan working hard to get their new HankMed business up and running while also preparing for the upcoming Fourth of July festivities. Their plans for a quiet holiday are interrupted when an unexpected guest arrives on their doorstep – Boris. Boris has brought his ill cousin, Dmitri, who needs medical attention and cannot risk going to a hospital due to his illegal status. Hank must use his expertise to diagnose and treat Dmitri while also keeping his condition a secret from the authorities.

Meanwhile, Divya is dealing with her own medical issues and must face the consequences of keeping them hidden from her friends and family. She is struggling with the decision to tell her fiancé, Raj, about her pregnancy and grappling with the fear of losing her job if her condition is revealed. Alongside this, Hank's ex-girlfriend, Jill, returns to the Hamptons with her new boyfriend, but seems to still have feelings for Hank.

The episode also explores the budding romantic relationship between Evan and Paige and their struggles to balance their work life with their personal life. Small town politics collide with HankMed business when Evan runs for and wins a position on the town council. As he tries to balance his new role with his responsibilities at HankMed, Evan quickly realizes that the position comes with unexpected strings attached.
