Man v. Food season 5 episode 21 is titled Omaha, and it follows the host, Casey Webb, as he travels to Nebraska to take on some of the city's biggest and best culinary challenges. In this episode, Casey visits several iconic restaurants in Omaha, each with a unique dish that is sure to test his limits.

The first stop on Casey's Omaha tour is at a restaurant called Block 16. Here, he tries the famous Croque Garcon, a massive sandwich made up of pork belly, smoked ham, gruyere cheese, and a fried egg, all piled high on grilled sourdough bread. This sandwich isn't just big in size, it's also big on flavor, and it's clear from Casey's reaction that this challenge is going to be tough.

Next up, Casey heads over to the Blackstone Meatball restaurant to take on their signature dish, the Big O meatball. This meatball weighs in at over a pound, and it's made up of a mixture of ground beef, pork, and Italian sausage. The meatball is served over a bed of spaghetti and topped with a generous helping of marinara sauce. Casey's task is to finish the entire dish in under 30 minutes, and it's clear from his reaction that this might be his toughest challenge yet.

After finishing the Big O meatball, Casey takes a break from the heavy eating to check out a local brewery, Infusion Brewing Company. Here, he samples some of the local craft beers and learns about the brewing process. It's a refreshing change of pace from the intense eating challenges, and it's clear that Casey is enjoying himself.

Next on the list is the Crescent Moon Ale House, where Casey takes on the Reubenator challenge. This challenge involves eating a giant Reuben sandwich that weighs in at over 5 pounds. The sandwich is made up of corned beef, sauerkraut, swiss cheese, and Thousand Island dressing, all piled high on rye bread. The challenge requires Casey to finish the entire sandwich in under an hour, and it's clear that he's going to have to dig deep to complete this one.

The final stop on Casey's Omaha tour is at the Block 16 Poutine restaurant, where he takes on the restaurant's signature dish, the Duck Fat Poutine. This dish is a French-Canadian classic, made with french fries, cheese curds, and gravy. But at Block 16, they take it to a whole new level by using duck fat to fry the potatoes and a smoked pork gravy. This challenge requires Casey to finish a massive bowl of poutine in under 30 minutes, and it's clear that he's feeling the pressure.

Throughout the episode, we see Casey's determination to complete each challenge, despite the difficultly of each task. He's a likable host with a great personality, and it's clear that he's having a lot of fun despite the intensity of the challenges. The restaurants featured in this episode all have their own unique character and charm, and the food itself is mouthwatering and inventive.
