Behind the White Tower is a Korean medical drama that tells about the power struggles and friendships within a busy, working hospital. The show focuses on the two main characters, named Jang Joon Hyeok and Choi Do Young. Both are doctors within the same hospital. However, this is where their similarities end. Do Young and Joon Hyeok have very different views about their careers and their patients.

Do Young is a very dedicated doctor. Above all, he cares about helping people. He strives to always to do his best to treat each of his patients with care. Although the world of the hospital is full of internal power struggles, Do Young has tried his best to avoid these conflicts. He has never sought promotions because he knows that will not help him treat his patients.

Joon Hyeok is the exact opposite of Do Young. Above all, he wants to climb the social and political ladder at the hospital. His biggest dream is to become the next department head of the hospital. Unfortunately, although he is technically a skilled doctor, Joon Hyeok's ambitions often get in the way of his job. He is careless when treating patients. This leads to extreme problems when patients begin to claim that they are not being treated correctly.

When a new doctor named Noh Min Guk joins the team, matters become even more complicated for Do Young and Joon Hyeok. It seems that Min Guk will be the one to take over as department head, ruining Joon Hyeok's dream. In addition, the current department head seems to want Min Guk to marry his daughter, Lee Yoon Jin. However, Yoon Jin takes matters into her own hands, and when trouble brews, she and Do Young band together. This causes old resentments and new troubles to stir. Both Do Young and Joon Hyeok must reevaluate their jobs and do what they feel is right.
