ABC channel's 'Extreme Makeover: Weight Loss Edition' is the latest addition to the Extreme Makeover reality series. Instead of revamping a home or bestowing cosmetic surgery on a chosen candidate, the focus of the show is a makeover on a severely obese participant who is ready to make a change to their unhealthy ways of living. The new series accepts the duty of helping these participants to alter their lifestyles and become fit and healthy.

The task of leading participants on the path to a healthy life style is left to Chris Powell, a fitness trainer who gained recognition by helping a client reach the highest recorded weight loss in the shortest period of time, via natural means. On the show, Powell not only provides the fitness training, but also offers guidance and support through the obstacles that arise during the twelve-month challenge. In addition to changing one's eating and fitness habits, the show also focuses on emotional issues that have contributed to unhealthy habits. While a psychologist assists the client in dealing with issues through discussion, Powell teaches the client how to release negative energies through exercise.

The show is paced over a 365-day period, which is broken down into three 4-month phases. A weight loss goal is set for each phase, with a reward given if each goal is met. Phase 1 is boot camp at the California Health and Longevity Institute, where Powell and a nutritionist educate the client with all the skills they need to achieve their health and fitness goals. Shortly after, client returns home to put these skills to the test through the remaining phases. Tools, such as exercise equipment and nutritious foods, are set up in the client's home to aid in the weight loss process.

All phases are shown in detail to the viewer, ending with a weigh in to see the participant's progress. The final phase culminates with a chosen plastic surgery, such as excess skin removal and a 'big reveal' party for family and friends to see results of the yearlong journey.
